Acknowledge On behalf of the contractor(Required)
On behalf of the contractor, I am authorized to acknowledge the following safety briefing and agree to abide by all of the items within: Failure to follow these may result in disciplinary action or removal from project. General: • All sub-contractors will comply with all Credence policies and procedures. Please refer to the Credence Health & Safety Manual for a complete listing of policies and procedures. • All sub-contractors must be pre-approved for work on Credence jobsites using the Credence Sub-Contractor Pre-Qualification system. • All sub-contractor employees must complete a Credence sub-contractor orientation before starting work onsite. This orientation is available online or in person. • ZERO TOLERANCE for consuming or being in possession of drugs and/or alcohol on any Credence Jobsite. • Contractors may be subjected to pre-access/post-incident drug & alcohol testing while working for Credence. • Harassment and bullying will not be tolerated. • Credence Construction must receive a copy of all SDS sheets for intended project products before arrival on site. • Theft and vandalism of Credence property will not be tolerated. • Smoking is only allowed in designated smoking areas. • Single action lighters are not allowed anytime while on any job site. (BIC Lighters) • All strings on “bunny hugs” or “hoodies” must be removed in order to be allowed on the job site. • Respirators must be fit tested using an approved method. • NO jewellery allowed except detachable tear away medical alert bracelets and stud type earrings. • ALL Incidents will be reported Immediately to Credence Supervisor. • Manual lifting of 50lbs or more will not be performed without assistance. • If working alone, you must be in contact with someone every 15 minutes. • All Lock Out Tag Out must be coordinated through a Credence Supervisor before starting work. • All legislation must be followed for the jurisdiction which work is being performed. 1. Paperwork: • TASC Cards (FLRA) must be completed for each task that is performed. • Toolbox Talks will be completed daily by each team. • Safety Meetings will be conducted weekly with entire crew. • Worksite Safety Inspections will be conducted weekly. • Emergency Response Plan must be developed & posted. 2. Training: • Onsite Orientations are to be scheduled 48hrs before arriving on site. • All contractor, sub-contractor certs must be provided 48hrs before arriving on site. • Proof of Fall Protection training is required before working at heights. • Proof of Confined Space Training is required before entering or attending a confined space. • All trade certifications must be provided. • SCOT/CSTS Training or equivalent is required for all crew members. • Proof of Powered Mobile Equipment training (forklift, JLG etc.) is required before operating any piece of PME. 3. Working at Heights: • Workers must be tied off when feet are at or above the 4ft level. • Non-compliance will result in removal from site. There are no exceptions to this rule. • All equipment must be inspected before each use. • Tool Lanyards must be used when working at heights or any other means of securing items from falling. • Secure hard hats to person with tethers when working at heights. • Keep all material 3ft from leading edge. • Secure all objects when working on elevated surfaces. • Tape and tag off area below. • Do not hang objects over guardrails. • Rescue plan must be in place for all fall protection situations. 4. PPE: • Hard Hat – CSA Approved • Safety Glasses – CSA Approved (NO contact lenses allowed) (Full seal may be required on some sites) • Safety Footwear – CSA Approved, 8-inch Ankle, laced all the way to top. • Gloves – 100% of the time. NO economy gloves. Proper glove for task. • High Vis – 2 or 4 Inch Striping (if 2” striping, it must be integrated into clothing – no suspenders) • Earplugs – To be used when sound level is over 85dBA or workers need to raise their voice. 5. PME: • Equipment check sheets must be filled out daily for each piece of equipment being operated. • Feet must remain on the floor of man baskets at ALL times. • Handrail cannot be used as a step at any time. • Proof of Powered Mobile Equipment training (forklift, JLG etc.) is required before operating any piece of PME. • Need 100% tie off in any Scissor lift, AWP, or other type of personnel lifting device. 6. Credence Permits: • Hot Work – Must be approved by Credence and Client before any Hot Work is performed. • Confined Space – Must be approved by Credence and Client before any work is performed. • Live Power – Must be approved by Credence and qualified Credence electrician before work is performed. • Ground Disturbance – Must be approved by Credence before work is performed. • Lock Removal – Must be approved by Credence before lock is removed. 7. Admin: • All invoices must be emailed to: invoicing@credencegroup.ca Additional contact persons may be CC’d on the email, however if these invoices are not sent to invoicing@credencegroup.ca, they risk not being paid. • All subcontractors must be appropriately registered for sales taxes (GST and PST) in the province/territory/state where the work will be performed. • All subcontractors must be appropriately registered for Workers Compensation (WCB) in the province/territory/state where the work will be performed. • All sub-contractors shall be prepared to sign a statutory declaration to accompany invoice. (where applicable) • If your company uses SiteDocs for safety record management – please Auto-Share all completed safety forms on our project with safety@credencegroup.ca 8. Credence Cardinal Rules • Driving Safety • Work Permits • Working at Heights • Drugs & Alcohol • Lockout/Tagout • Powered Mobile Equipment • Unauthorized Entry into Red Flagged or Hard Barricaded areas • We will never overlook the breaking of a Cardinal Rule, it will always result in disciplinary action regardless of the person or situation. I have read and agree to the items presented above. I acknowledge that failing to follow these may result in disciplinary action or removal from project. I acknowledge that I will communicate these items to any staff that will be working on a Credence Project. Must be signed by an authorized/binding representative of your company.
